Vision Plan
PPO vision care benefits are administered by EyeMed. A link to the Benefits Summary is shown below. The EyeMed network includes many major chains, as well as independent optometrists and ophthalmologists. Provider information is available on the EyeMed website, and instructions on how to locate an EyeMed provider can be accessed using the link below. A claim form for services from a provider who is not part of the EyeMed network is also included.
